On June 14, 2022, the Taiwan Food and Drug Administration approved the use of the Moderna COVID-19 vaccine and the Pfizer-BioNTech COVID-19 (Pfizer) pediatric vaccine for children and adolescents. An 8-year-old patient in this case report received the Pfizer pediatric vaccine on June 16, 2022. Within a week of receiving the vaccination, the patient developed successive red and itchy rash lesions in the axillae and knee creases, which gradually expanded to involve the face and neck. The extent of the red and itchy rash lesions increased, the color became redder, and some lesions began to exhibit tissue exudation, accompanied by ulceration, crusting, desquamation, and eczematous changes, representing a variety of dermatological manifestations. Starting from October 19, the patient underwent traditional Chinese medicine treatment, focusing on heat-clearing, detoxification, and dampness-eliminating principles. The herbal prescription consisted of Ganlu Xiaodu Dan in combination with Xiao Feng San, with modifications. During the course of treatment, the skin manifestations, tissue exudation, and ulceration gradually improved. After one month of traditional Chinese medicine treatment, all lesions had completely healed, demonstrating the efficacy of traditional Chinese medicine in managing adverse reactions to vaccines. The use of Chinese herbal pharmacology in alignment with clinical mechanisms contributed to enhanced therapeutic effects and reduced severity.
