Sudden Sensorineural Hearing Loss treated with Acupuncture: A Case Report

【摘要】
78 歲婦女曾經因罹患右側乳癌接受手術之過去病史，目前無其他慢性病。
此次因右耳罹患突發性感音神經性聽力喪失，已於耳鼻喉科接受鼓膜內類固醇
注射治療3週，但症狀及純音聽力測驗結果未見改善。初診時，切診其右手關
部為滑脈、左手關部為弦動脈，經四診合參診斷其暴聾屬於肝氣鬱結、氣鬱化
火證型，治療原則為疏肝理氣、清熱泄火。以針刺治療疏通其少陽經脈，調節
氣機升降，局部取穴以患耳之耳門穴、聽宮穴、聽會穴、風池穴為主，遠端則
取足三里穴、合谷穴，藉此改善耳部周圍血液循環。病患在4次針刺治療後主
觀聽覺症狀進步迅速，純音聽力測驗亦有極大進展。本病例發表以針刺治療介
入耳鼻喉科常規治療無效之突發性感音神經性聽力喪失之經驗，可望提供替代
醫療之參考。

【Summary】
A 78-year-old woman with a history of surgery for right-sided breast cancer, but now without other chronic diseases. This time, she suffered from a sudden sensorineural hearing loss in his right ear, and then has been treated with intratympanic steroid injections at the Department of Otolaryngology for 3 weeks. However, there was no improvement in symptoms and the result of pure-tone audiometry test. At the first visit, the right hand was showing slippery pulse in the pulse diagnosis, and wiry-moving pulse on the left hand. The syndrome differentiation of TCM is that the depressed qi transforming into fire, Therefore, the treatment principles is to disperse stagnated liver qi and clearing heat-fire. Acupuncture treatment is used to dredge its Shaoyang meridians, adjust air engine. Acupoints like Ermen (SJ21)、 Tinggong(SI19)、 Tinghui(GB02)、 Fengchi(GB20) were applicated to improves blood circulation around the Affected ear. The patient’s subjective auditory symptoms and the pure-tone audiometry test had significant progress after 4 acupuncture sessions. Through the TCM therapy in treating with this case, we could provide the experience on alternative medicine to the treatment of Sudden sensorineural hearing loss that does not respond to conventional treatment.
