In this case, a 64-year-old man suffered from acute SAH, ICH ,SDH and IVH after traffic accident on 2021/08/06 . At the time of hospital admission, his GCS was 3 and brain CTA revealed A-com aneurysm. After receiving an acute operation of craniotomy for clipping of A-com aneurysm, his ICP was within normal range and consciousness began to improve. However, his respiratory status remained poor. He began weaning training program from 2021/08/20 and the spontaneous breathing trial has been failed on 2021/08/30. In order to improve his respiratory status, he was transferred to Respiratory Care Center (RCC) for further weaning training program on 2021/08/31. Due to the difficulty in weaning from mechanical ventilation, traditional Chinese medicine therapy has been consulted on 2021/09/02.The patient was diagnosed as having lung qi deficiency. After the prescription of acupuncture and Chinese herb powder in the strategy of nourishing heart qi and reinforcing the kidney qi, the patient successfully weaning from mechanical ventilation in one week. Following two weeks of treatment, he was then discharged from the hospital. This case explores the goals that traditional chinese medicine therapy in the strategy of nourishing heart qi and reinforcing the kidney qi may be helpful for difficulty in weaning from mechanical ventilation.
