【摘要】
因疫苗接種造成相關的肩部損傷(SIRVA)是一種更嚴重的疫苗接種後併發症,與肌肉注射疫苗後常見的局部痠痛相比,SIRVA 會出現肩部持續嚴重的疼痛和活動範圍受限。目前的理論是,疫苗注射到肩關節或三角肌下會導致發炎反應,並影響包括滑囊、肌腱和韌帶在內的周圍結構。新型冠狀病毒 COVID-19 全球疫情嚴峻下,疫苗施打是每位人民保護自己也保護他人的重要措施,疫苗接種政策盛行同時也使 SIRVA 的全球發病率呈上升趨勢。本案例是一名50歲男性,於11012月接種 covid-19疫苗後,開始右手上舉困難,上舉約60度三角肌處痛,影響肩關節活動度受限,症狀持續三個月未好轉,於1113月前往北榮傳統醫學部針灸治療後,肩關節活動度增加許多,恢復度明顯進步。希望這一有效案例能夠提供給中醫界作為臨床實踐的參考,並且在這全球疫情下,我們可以證明中醫能幫助且改善疫苗接種後的副作用。

Summary
Shoulder injury related to vaccine administration (SIRVA) is a more serious complication, comparing with common and localized soreness and pain following intramuscular vaccinations, and causing persistent pain and limited range of motion in the shoulder after the vaccination. The current theory is that inadvertent injection of the vaccine into the shoulder joint or subdeltoid bursa results in an inflammatory response and affects surrounding structures including the bursa, tendons, and ligaments. As the vaccination policy for preventing the spread of Severe Acute Respiratory Syndrome Coronavirus (SARS-COV2), it is believed that the incidence of SIRVA is increasing globally. We present a reported case of SIRVA following a mRNA COVID-19 vaccination. A 50-year-old male results in onset of right shoulder pain and limited range of motion that persists for three months after the vaccination. After receiving Chinese Medicine treatment, his symptoms improved significantly. This effective case is hoped to be provided to the Traditional Chinese Medicine practitioners as a reference for clinical practice. And under the global epidemic, we can conclude that Traditional Chinese Medicine can help the side effect after vaccination.
