A Case Report on Integrative Therapy with Acupuncture and Herbal Medicine for Malignant Wounds in Patients with Advanced Breast Cancer

【摘要】
研究對象為一名66歲女性患者,因腦血管疾病住院期間意外發現左側胸部大片潰瘍性傷口(約11 cm x 13 cm),並確診為末期乳癌,伴多處淋巴結及骨骼轉移(cT4cN3cM1),並同時患有右側內頸動脈M1段阻塞,伴隨大面積腦梗塞,接受靜脈血栓溶解療法(rt-PA)及減壓性顱骨切除術後,意識不清(E3V1M1),四肢癱瘓,僅能以微弱眨眼進行溝通。於病發三個月後開始會診中醫治療,治療方式包括水煎藥和針灸療法。患者的左側胸部傷口分布於乳頭上下及內側,初始表現為紫紅色,邊緣伴暗色結痂,中心鮮紅且容易滲血。在接受中西醫結合治療後,傷口滲血逐漸止住,結痂部位開始脫落,整體傷口呈現癒合的跡象。此外,患者並未出現明顯的併發症,整體狀況較治療前有所穩定。

【Summary】
This case involves a 66-year-old female patient who was hospitalized due to a cerebrovascular disease. During her hospitalization, a large ulcerative wound (approximately 11 cm × 13 cm) was incidentally discovered on her left chest. She was subsequently diagnosed with advanced-stage breast cancer, accompanied by multiple lymph node and bone metastases (cT4cN3cM1). Additionally, the patient suffered from an M1 segment occlusion of the right internal carotid artery with extensive cerebral infarction. After receiving intravenous thrombolytic therapy (rt-PA) and decompressive craniectomy, she presented with impaired consciousness (E3V1M1) and quadriplegia, with communication limited to weak blinking. Three months after the onset of her condition, she began Traditional Chinese Medicine (TCM) treatments, which included herbal decoctions and acupuncture therapy. The wound on her left chest, located around the upper and lower regions of the nipple and extending medially, initially appeared purple-red with dark crusted edges and a bright red, easily bleeding center. Following a combination of TCM and Western medicine treatments, the wound gradually stopped bleeding, and the crusted areas began to slough off, showing signs of healing. Moreover, the patient did not experience significant complications, and her overall condition showed stabilization compared to her pretreatment state.
